District NursesThe District Nurses care for patients in their own homes, this includes; managing and delivering care to patients with complex health needs, giving support and advice in relation to their ongoing treatment, wound management, palliative care, bereavement support and health promotion. Referral to the District Nursing team can be done by your GP, Practice Nurse or Hospital Clinician. |
Community MatronsOur community matrons are registered nurses who provide regular contact for housebound patients who have long-term medical conditions. The community matrons see patients at home and also those who live in nursing and residential homes. Referral to community matrons is via the GP, Hospital Consultant or Clinical Nurse Specialist. |
MidwivesThe midwives hold a weekly antenatal clinic here in the surgery on Thursday mornings. Appointments for this clinic can be booked by reception. |
Health VisitorsHealth Visitors work in people's homes, community health centres, Children's Centres and GP surgeries supporting parents in the promotion of health and wellbeing of all pre-school aged children across Liverpool. All children and families with children 5 and under receive this service. |
