How to Register

Please note: If your address falls outside our catchment area, we may not be able to accept your registration.

 
 

There are 2 forms to complete, a GMS1 form and a Health Questionnaire, both of these are required to register.

The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery. The Health Questionnaire will ask you to give important health and lifestyle information, as well as any specific needs, which will enable us to provide the best service possible for you.

 
 
When completing these forms, please ensure that you read the information carefully and provide accurate information.
 
 

Once you have completed and signed the forms, please bring them to the surgery to be checked, along with the following ID:

  • Photo Identification (e.g. driving licence or passport)
  • Proof of address (e.g. utility bill or bank statement)

Please note: failure to provide valid ID will not prevent you from being registered.

 
 

Once you have returned your registration forms and your ID has been checked, you will need to book an appointment for a new patient health check with a nurse or HCA in order to complete the registration process. Please book this appointment with reception as soon as possible, to ensure your registration is completed.
