How to Register
1) Check that you live within the Practice's Catchment Area
Please use the map and postcode check below to determine if you live within our catchment area.
Please note: If your address falls outside our catchment area, we will not be able to accept your registration.
2) Complete Registration and Health Check Forms
There are 2 forms to complete, a GMS1 form and a Health Questionnaire, both of these are required to register.
If you are registering a newborn baby, you only need to fill in a GMS1 form and a Baby Ethnicity Form.
The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery. The Health Questionnaire will ask you to give important health and lifestyle information, as well as any specific needs, which will enable us to provide the best service possible for you.

When completing these forms, please ensure that you read the information carefully and ensure that you provide accurate information.
Unfortunately the registration forms can not be sent back electronically to the practice as we require a signature to register you as a patient.
3) Placement on the Practice List
Once you have completed and signed the forms, please bring them to the surgery to be checked, along with the following ID:
- Photo Identification (e.g. driving licence or passport)
- Proof of address (e.g. utility bill or bank statement)
Please note: we will not accept a completed registration form without the above identification.
4) New Patient Health Check
Once you have returned your registration forms and your ID has been checked, you will need to book an appointment for a new patient health check with a nurse or HCA in order to complete the registration process. Please book this appointment with reception as soon as possible, to ensure your registration is completed.
